Night 4's Intruder kills anyone living alone, while the Vigilante returns on Nights 8, 10, and 12 to test your own body for symptoms.

Typically the first knock. Never a Visitor and essential for ensuring you are not alone on Night 4.
Appears Day 1 evening; reappears later with additional dialogue.
Neighbor's Daughter
Frightened child with red, tearful eyes (a false symptom). Turning her away locks you out of an achievement and story beats.
Visits early and returns on Night 4 searching for her father.
Burned Firefighter
Soot-covered survivor who hands over a FEMA Notice so you can control future raids.
Guaranteed arrival on Night 7; will not enter if the Surgeon is present.
Guaranteed Visitors
These entities never pass as human, yet some unlock secret content or endings.
The Intruder
Shirtless pale figure who murders isolated players and returns multiple nights to pressure-test your shelter.
Mandatory encounter on Night 4 and again on Nights 5, 9, and 11.
Cat Lady
Erratic visitor carrying a cat; the only way to unlock the basement for the Mushroom and Egghead questlines.
Appears Night 4 accompanied by her cat—accepting her grants cat food insurance.
Cashier Lady
Harmless Visitor who seems unaware of her condition, posing a moral dilemma for pacifist runs.
Arrives during the first evenings and calmly requests shelter.
Randomised Identities
Wild cards that require multi-trait verification each playthrough.
Twin Sisters
Individually random identities. One may be human while the other manifests Visitor symptoms.
Often knock together mid-game—assess each separately before granting entry.
Fortune Teller
Provides occult clues and can initiate the cult storyline, yet may secretly be a Visitor with red eyes.
Cycles through the later nights; identity changes between runs.
Man in the Suit
Corporate survivor whose armpit fungus only appears when he's a Visitor. Conflicts with the Nun and Cat Lady.
Shows up after Night 6 depending on occupancy; double-check photos and armpits.
