Who they are
Death Cult Leader is a middle-aged man with a long thin face, wide reflective eyes, and a dark buttoned robe that reads as clerical. He speaks like someone who has already decided the rite is mercy. The character article calls the species "human (highly likely)" and does not put him on the guest roster. The people he leaves behind are the roster entry: Death Cult Peons, one row, three bodies, all guaranteed human, in the office from night 8.
The route is patience. Keep all three alive until he comes back, and do not let FEMA Agent remove them for testing. Then accept the invitation. The ending is May Death Cleanse Us of Our Sins, achievement ach_30 on the peons' roster row. There is no spoke page for it yet. Refusing, or losing one peon, drops the finale. Peon number two is the one the article singles out as afraid when the gun comes up. That fear does not make him a Visitor.
The three wear indigo robes, with dirty hands and red sclera. Those marks are called out so you do not spend a clue day "catching" a fixed human. Angry Guy talks about a wife who joined the cult. He is a separate randomized guest and not a substitute for one of the three.
This ritual is not Death in the basement. Embrace The Inevitable is the tunnel route. The leader's ending is the invitation. Players who want both have to do them on different runs, because the steps fight over the cat-guest days, the FEMA visits, and the final night.

FAQ
Is the Death Cult Leader a guest?
No. The three peons are the roster entry. He returns later with the ritual.
How do you get the cult ending?
Keep all three peons alive, away from FEMA, until he returns, and accept. Achievement ach_30 matches that ending.
Are the cultists human?
Yes. The roster locks the peons as guaranteed humans. Dirty hands and red eyes are not a reason to shoot them.
Is this the same ending as jumping into the hole?
No. The hole is Embrace The Inevitable. The robe and the invitation are the cult finale.
